Here are some important Definitions and instructions:
1. Payor (Parent with less parenting time usually)
2. Custodial Parent (Parent who has most parenting time)
3. Payor’s Other Obligations (Child Support actually paid for other children ordered by a court prior to this matter)
4. Custodial Parent’s Other Obligations (Child Support actually paid for other children ordered by a court prior to this matter)
5. Number of Children (shared by the Payor and Custodial Parent)
6. Childcare Costs paid for child(ren) shared by the Payor and Custodial Parent
7. Health Insurance Costs paid for child(ren) shared by the Payor and Custodial Parent

*Income and Expense amounts must be the monthly amount.

**The “Currently Paid Support”: Only fill this in if child support is already being paid for child(ren) shared by the Payor and Custodial Parent. This will let you know if a modification of child support would be useful in the case depending on whether you are the Payor or Custodial Parent.

Parenting Time Adjustment (Parenting days must equal 365 days total for both parents
